Sorry guys, I should have had it scanned and uploaded here. Charlie was asking about it too recently. Scanner's knacked, so my wife's getting it done for me. Will either stick it on the site here, or mail copies to anyone who wants one.

Will Dave sent me a scan approx 10 days ago - quite a large pdf file but if you provide me with details I will send it to you or I can print off and post.
Charlie - provide me with details and I'll send over a copy.
